Correct me if I'm wrong but fantasy Johnny in this clip @ about 3:00 looks a lot like Mark Hapka aka Dr Nathan Horton. For some reason I found this amusing.

Someone the other day had a clip where Alison Sweeney played flashback Kayla.

Just wondering what other fantasy / flashback coincidences have occurred?

It WAS him. Back when he got the role of nathan, it was mentioned he had played fantasy Johnny in that sequence.

Ali Sweeney played Young Adrienne, not Kayla, in a flashback in 1987. Check at around the 2:50 mark.
