FBI agent Rafael (Rafe) Hernandez first appeared in Salem in the fall of 2008. He came to give information to Sami Brady, (and her guard) who was in the witness protection program because of witnessing the murder of the Salem mayor on the DiMera’s doorstep. However, once he arrived, he found that Sami had given sleeping pills to her guard. When Sami's guard awoke, she resigned her post and left Rafe to guard Sami.
Rafe and Sami butted heads in the beginning, but ultimately formed a friendly relationship as Sami related details of her personal life. Rafe stayed fairly quiet about his personal life, but did mention that he spent a lot of time at the local convent when he was younger because he was a bit of a troubled youth. Rafe was one of the few people who knew that Sami was pregnant while in witness protection. He also knew that EJ was the father of Sami's baby, but went along with Sami's plan to hide Grace's paternity. Rafe was curious about what was so bad about EJ that Sami would feel compelled to hide her pregnancy so he broke into the DiMera mansion to spy on EJ. While he was out he was attacked by the man who killed the mayor, as the killer tried to get him to tell him where Sami was hiding. Rafe made it back to the safe house where Sami nursed his knife wound. Their location was compromised after Sami’s first guard, Hildy, was murdered by the mayor’s killer, forcing Rafe and Sami to go to the convent to hide. While at the convent Sami gave birth to her baby at the local clinic and brought it back to the convent where Rafe bonded with baby Grace and Sami asked him to be Grace’s Godfather.
After the killer was caught and Sami was out of hiding, Rafe wanted to stay in Sami’s life, realizing he had strong feelings for her. Rafe was let go from the FBI for breaching protocol while protecting Sami in the witness protection program. He tried to apply for a job with the Salem PD, wanting to build a future with Sami. When the Salem PD could not hire him, he decided to stay in Salem anyway to be with Sami and Grace. Later, he also reconnected with his formerly incarcerated sister, Arianna.
Despite Arianna's protests against his relationship with Sami, Rafe moved forward with her. He asked if he could adopt Sami's baby, Grace, and Sami agreed. But, before the adoption could be legalized Grace died of bacterial meningitis. Rafe was devastated.
In the wake of Grace’s death, Sami and Rafe decided to move in together. While they were unpacking his things Sami came across an engagement ring. Rafe explained to Sami he was engaged to Emily Hudson but she died tragically in an accident on the day of their wedding. Rafe was evasive about further details of the accident, but it was revealed later that it involved his sister Arianna and Rafe may have covered something up. Sami questioned Rafe further but he never revealed anymore to her about his former fiancé.
Emily's sister, Meredith, returned to Salem, convinced Rafe was to blame for her sister’s death. Unseen in Sami’s apartment, Meredith knocked Rafe out, kidnapped him and held him captive in the basement of her parent’s old house. She was planning on walling him up behind a brick wall, leaving him to die. Arianna became suspicious of Rafe’s whereabouts and knowing Meredith was in town, went to the house to see if she had seen Rafe. Meredith realized Arianna was suspicious so she hit Rafe in the head with a brick and threw him into the river.
Luckily, Carly Manning came along, fished Rafe out of the river, and nursed him back to health on her boat as she made her way back to Salem. Rafe returned to Salem and reunited with Sami, who was delighted by his return. He never told Sami of the danger he was in. Rafe began to investigate Nicole Walker, who had befriended Sami, her once arch nemesis. Nicole’s strange behavior towards Sami and her devastation over the death of Grace made Rafe suspicious. After a few weeks of investigation he realized that Nicole had somehow switched Sami’s baby with another girl's, leading Rafe to believe Grace was not Sami’s biological baby, but instead was Sydney, the baby who Nicole was raising with EJ. He snuck DNA samples from Sydney and Sami and had them secretly analyzed at the FBI. When he learned they were a match, he told Sami what Nicole had done. Sami got her daughter Sydney back.
But, Sami and Rafe's reunion with Sydney was short lived. Nicole, who was desperate to keep “her” daughter kidnapped Sydney from the DiMera mansion and fled to Cleveland. While on the run, Anna DiMera caught up with Nicole, knocked her out and kidnapped Sydney. Anna sent Sami ransom notes and instructed Sami not to involve the police. Sami told Rafe about the first ransom note, which resulted in a botched ransom exchange, After that Sami kept future letters from Rafe and confided in EJ. When Rafe found out, he was upset that Sami would trust EJ over him and broke up with her. He could not bear to watch Sami lean on and trust EJ. Eventually EJ brokered a ransom deal and returned Sydney to Sami, only after everyone was lead to believe Sydney was dead.
Rafe still loved Sami so he continued to investigate Sydney's kidnapping, as he was not convinced that Anna was working alone. Rafe suspected that EJ was behind the kidnapping, that he did it so that he could play hero to Sami by bringing Sydney back, thus ingratiating himself to Sami. Rafe found Anna on a tropical island, tried to get Anna to tell him who hired her. Anna was poisoned just as she was about to reveal the truth and was rushed to the hospital. Rafe was arrested for the poisoning and thrown in a foreign jail. He broke out of the jail after meeting up with Shane Donovan and made his way back to the hospital to question Anna. When he arrived, Anna was gone. A disappointed Rafe returned to Salem.
Now Rafe set his sights on Nicole, after he overhearing a suspicious conversation between EJ and Nicole. Positive that Nicole had proof that EJ was the one who paid Anna to kidnap Sydney, he learned from Victor how to get Nicole to trust him, and thus managed to get invited to Nicole's apt. When Nicole fell asleep, he searched the apt., found a recording of EJ telling his father that he paid Anna to kidnap Sydney. He promised Nicole immunity and protection if she went on record confirming the conversation on the recording. Nicole had no choice but to agree, but was terrified of EJ’s revenge, but Rafe put FBI agents with Nicole to protect her from EJ..
Sami, in the meantime, was trying to find Rafe, and spotted him kissing Nicole. Not knowing he was doing it to get information, a jealous Sami went back to the DiMera house where she was now living, told EJ, and agreed to marry EJ immediately. When Rafe heard about the wedding he rushed over to the DiMera mansion to stop it. He arrived just in time, stopping the wedding, playing the recording for Sami.She called it off with EJ, left with Rafe, horrified that EJ would tell her that her daughter ws dead. Rafe & Sami reconciled and Sami was very grateful to Rafe for exposing EJ’s lies. They spent the night together and Rafe proposed to Sami. She accepted. However, what Rafe didn't know was that later, Sami snuck out of his apartment in the middle of the night to go confront EJ after Kate called to tell her EJ was going to take her kids and disappear. She found him passed out with booze in one hand and a gun in the other. She shot him in the head with his own gun and threw the gun into the river.
The next morning, Bo questioned Sami about EJ's shooting. Rafe insisted that Sami was with him all night. Rafe suspected that Sami’s son Will shot EJ. He resigned from the FBI so that he would not have any conflicts and would be able to protect Will from an attempted murder rap. Sami could not let Rafe believe Will was the shooter, so she eventually told Rafe the truth.... that she was the one who shot EJ. Rafe vowed to always protect Sami and not let her go to prison for shooting EJ.
